Grave Marker Request

I confirm that I am legally authorized to make this request on behalf of the deceased’s family. I consent to the Islamic Center of Connecticut coordinating with the cemetery for the placement of the grave marker. I understand that the marker must comply with cemetery rules and Islamic guidelines, and that any costs associated with the marker are my responsibility.
